"Greatest Supply Chain Disasters"

Courtesy of Jason Busch I saw this listing of 11 "disasters"

Knowing executives in at least 3 of the citations, reality is far more complex than what the blurbs describe. The end result is conservative, over tested implementations are now the norm. I would estimate at 25%  project cost in additional testing. If you add all that up across thousands of projects, it amounts to a much larger disaster.

Remember the Alamo is a rallying cry. Remember Hershey is a slow down cry - and it costs a lot.

The industry is screaming for streamlined, better testing and release management disciplines. Far more statistical representation of risks, rather than gun to the head threats about past failures.
